POSITION SUMMARY:
Catholic Charities is working closely with the Juvenile Probation Department to provide Mental Health Services, Case Management Services, Vocational and Educational Services, and Pro-Social activities to youth on probation.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Provides Clinical Case Management services
Meet with families in the school, community or in the home to empower clients
Formulate, implement and monitor individual client and family assessments and treatment planning
Conduct follow-ups and monitor service delivery and client progress
Ensure that all required documentation is properly completed within the required timeframes
Transfer family and service documentation to electronic data systems as necessary, including AWARDS (electronic health system) and other required Medi-Cal based tracking systems
Maintains a caseload of 17
Lead Rehab and/or Therapy Groups as needed
Other responsibilities as assigned by supervisor
QUALIFICATIONS:
EDUCATION AND EXPERIENCE:
MA or MS in counseling, psychology, social work or related field (REQUIRED)
Board of Behavioral Sciences registration as MFT Intern, ASW or PCC Intern (REQUIRED)
Bilingual Spanish/English (REQUIRED)
Certified in Triple P Teen (Recommended)
Counseling experience in a community based setting (Recommended)
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
Effective verbal and written communication skills
Understanding of Medi-Cal eligible service provision and billing requirements
Understanding of and sensitivity toward economic and cultural issues in diverse communities
Ability to work independently with minimal supervision
Demonstrated computer literacy (MS Word, Excel, and Internet usage)
Ability to pass certification requirements for CANS assessment 90 days of hire (training and support will be provided)
Team oriented with strong interpersonal and organizational skills
PHYSICAL, EMOTIONAL, AND INTELLECTUAL REQUIREMENTS:
Thrive in a fast paced, diverse and dynamic environment
The service is sometimes physical, requiring setting up for events which may require transporting or lifting cumbersome materials, e.g., easels, flip charts, refreshments, brochures, chairs, tables, etc.
Must be able to report to service site on a regular and reliable basis
Able to travel between sites and to attend offsite events throughout the county, as appropriate
OTHER QUALIFICATIONS:
Must be at least 18 years of age
Must be a U.S. citizen or lawful permanent resident
Valid California driver’s license, automobile for local travel and client transport, proof of current auto insurance
Fingerprinting and background check as required by the DOJ, and DMV clearance
TB test
Provide proof of COVID-19 vaccination status
