Schedule: Tuesday and/or Wednesday, Thursday 10:30-7pm. Monday and Friday can be flexible.

Position Description: This is an exciting opportunity to join our team as we implement an innovative mental health program – Axis Bridge!

Axis Bridge is new model of mental health care which will provide stabilization and short-term treatment to individuals within the Tri-Valley regardless of their type of insurance or ability to pay. It will act as a central location for individuals and families to access when they are unable to obtain immediate mental health services.

We are seeking a therapist with strong clinical skills and a passion for innovation to join our team or therapists, care coordinator and psychiatrist who will make up this program.

Qualifications:

  • Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Psychologist (PsyD/PhD).

Required Competencies:

  • Strong assessment and diagnostic skills.
  • *Loan repayment application - up to $50k for the first 2 years (through the National Health Service Corp Program).
  • Experience providing brief intervention and short-term counseling.
  • Experience working in higher levels of care, or with patients in acute need.
  • Ability to independently decision make in urgent situations, while also working as a part of a team.
  • Experience working with individuals who have co-morbid presentations.
  • Ability to utilize evidence based approaches, while also tailoring approach to client’s specific needs.
  • Experience working in a multi-disciplinary team in a medical setting. Good communication skills.
  • Strong documentation skills.
  • Computer knowledge and ability to type is required. Previous experience using electronic health records is a plus.
  • **Up to $15,000 sign-on bonus for direct hires providers who work 0.8-1.0 FTE.

*As a primary care provider with Axis Community Health, you may qualify for up to $50,000 in loan repayment in exchange for a two-year service commitment

*Employees who are re-hires, internal transfers, internal promotions, consultants, contractors, or temporary staff are not eligible for this bonus, Providers must complete 90 days of active employment with Axis and maintain FTE eligibility to qualify for this bonus. Direct hires will receive a $15,000 bonus. Providers that are hired as an employee through an agency will receive a $5,000 sign-on bonus,

Essential Duties:

  • Address the needs of patients who present to the program with an urgent need as available. There will be no specific productivity expectations or ongoing caseload within the Bridge program. However, clinicians will hold a small panel of cases in the general clinic.
  • Work in collaboration with the psychiatrist and care coordinator to stabilize patients presenting to the program.
  • Collaborate with outside providers who will be providing longer-term care and ensure continuity of care as possible.
  • Complete documentation using the electronic health record in a timely fashion while adhering to related laws and regulations for documentation.
  • Attend weekly staff meetings.
  • Provide all services in accordance with accepted standards of care and Axis protocols within the parameters of agency contractual requirements.

Physical Demands and Working Conditions:

Work is normally performed in a typical office work environment. While performing the responsibilities of this position, the individual is required to have ambulatory skills sufficient to visit various parts of the organization. Requires the ability to reach with hands and arms, climb stairs and occasionally lift and/or move up to 30lbs. Requires sitting and standing for extensive periods of time, operating a computer keyboard, telephone and other equipment for extended periods of time. Travel to other Axis health centers and other local travel will be required.
